A testicular cancer caregiver and Master of Clinical Social Work student at Azusa Pacific University is requesting help with her research project.
The purpose of this study is to conduct an evaluation of how familiar testicular cancer patients, survivors, and caregivers are the with services provided by clinical social workers. I would appreciate it if you would please take the time to complete a survey to help determine how effective current education is regarding services provided by social workers to cancer patients.
Your participation is voluntary. It involves completing a 15-minute survey, then reading a digital informational pamphlet about services provided by social workers, and lastly completing a post survey to see what information may have been gained and likely utilized. You may withdraw from the survey at any time. Your identity is safeguarded by an anonymous setting on the survey site. All data will be safeguarded, anonymous, and shared by themed groupings of patients, survivors, and caregivers. The individual data will be aggregated by identified groups, to discover themes.
This project has been reviewed to determine that participants’ rights are safeguarded and there is minimal risk or discomfort associated with the completion of this study. Your responses will be kept strictly confidential, and stored in a protected digital file or hardcopy locked files. Information will only be accessible to the researcher. The project will be presented as aggregated data to the administration for purposes of program improvement.
