With that much yolk sac, I imagine you had elevated AFP before surgery. Is it back to normal yet? That would be a determining factor in what treatment option to choose.

Thanks for your reply. When I go for medical checkup last July and found that my AFP level was 200. My family doctor sent me to a herpetologist and liver transplant surgeon because I had a Hepatitis B in my blood. Spent 6 months of checking, testing for my liver but couldn't find any tumor in my liver and they still want to do more test for my liver even that my liver functions test is OK. I only got elevated AFP and the rest are normal. My AFP elevated from 200 to 500, 900 and 1200 in 6 months and the herpetologist told me that may be I'm OK. I, myself suspected that may be the tumor locates somewhere else then I searched on the internet for AFP and found that the tumor may be in my testis. I feel the left one really hard and double the size of the right one. Went to see the urologist and he told me that I have TC after he draws my blood and send me to CAT scan. I got negative (clear) results for my CAT scan on my lung, abdomen, and pelvis. (Good news)

My urologist suggested me to see the oncologist for second opinion and he also suggests me to wait for 2 or 3 more weeks to test my AFP. If it is back to normal, he suggested that I can do surveillance protocol.

I have had an infection on my left testis back in September 2004 and the urologist (different urologist) told me that it’s only an infection after I took some antibiotics medicine from him. I think that is the beginning sign for my TC. After this infection, my left one always a little harder and larger than the other one but I really not paying any attention until I found out from internet that elevated AFP came from liver and testis.

That was my story. Lucky I found it out by myself. I’m still angry with the herpetologist and the liver transplant surgeon. I guest that they knew I have a tumor somewhere else but they want their business and that why they keep sending me to do lots of useless tests.

Good day to all. I just found this site and have found the information and the posts very helpful to me. I’m seeking some advises from you guys for my case.

Diagnosed TC 12/26/05, Left I/0 1/05/06

Tumor Staging:
Tumor type: Mixed germ cell tumor (75% Yolk Sac, 25% Mature Teratoma) (Nonseminoma)
Tumor Size: 6.5 cm (Wow )
Tumor Pathology stage: PT2
Regional lymph nodes: Negative (N0)
Distant metastasis: Unknown
Site of distant metastasis: Unknown
Stage grouping: IB
Resection margin: Negative

Anatomic Pathology Diagnosis:
-Tumor invades into epididymis
-Angiolymphatic invasion is present ( This may change my decision from surveillance to RLPND)
-Non-lesional testis is a mixture of atrophic changes and sclerotic fibrosis
-Spermatic cord margin is free of tumor

Should I choose surveillance or RLPND?
I’m living Houston with good medical center and doctors.
