    The surgeon removed big conglomerate of lymph nodes, histology found teratoma and cancer inside a blood vessel. Surgeon recons it is a vessel, teraroma has grown to feed utself, as he did not dissect blood vessels. So we are talking cancer inside teratoma, which is extremely rare. Has anyone heard about such things?

      What are the symptoms of Chylous ascites? My husband had RPLND ten days ago and has very strong pain that appared this night in the place where lymph nodes mass used to be? He did not have such a bad pain before, Chylous ascites how can they be diagnised?

        Certainly could be. I would call your on-call physician and talk with them.
        6/5/15: bHCG 27,AFP 8.66, LDH 361, 5.6cm lymph node - Stage IIC
        6/16/15: Left I/O 85% EC, 10% chorio, 5% yolk sac opinion 2 (mayo) 90% EC, 10% yolk sac
        7/7/15: bHCG 56, AFP 42, LDH 322
        7/13/15: begin 4xEP, end 9/18/15
        10/1/15: bloodwork normal, ct scan shows 2 lymph nodes 1.0cm
        10/26/15: 2nd opinion on CT results - lymph nodes normal. Surveillance!
        4/6/16: 1.7cm X 1.5cm lymph node found with markers normal.
        4/20/16: RPLND @ IU - teratoma only!
        10/22/19: all clears up to this date!

          How long does it take AFP to notmalise after RPLND? 1st april 55, 6th of April RPLND, 24th of April 29?

              Te half life of AFP is about one week.
              Jan, 1975: Right I/O, followed by RPLND
              Dec, 2009: Left I/O, followed by 3xBEP

                Histology came back:
                They found necrotic tissue in lymph nodes and the extracted mass.
                They found Embryonal Carcinoma in every sample of the tissue, surrounding extracted mass of lymph nodes. Surgeon said it was the first time in his practice and he is very experienced.
                AFP dropped to 29 last week, RPLND was on the 6th of April. We will have another AFP measurement tomorrow or today later in the day.
                I understand that this is bad news

                  Well the good thing is that they removed a lot of the infected nodes. This should help contain the spread and hopefully, any adjuvant chemo will finish off anything that is left.

                    My husband starts IEP soon, as the marker climbs slowly. Dr E. Is not optimistic at all.

                      I'm very sorry to hear your husband's markers are rising again. Is IEP Ifosfamide, Etoposide and Cisplatin? Sending positive thoughts!
                      Sep. 2015: Diagnosed with large primary mediastinal GCT and lung met.
                      Sep.-Nov. 2015: 1xPEI/VIP, 1xBEP, 2xTIP, markers normal after chemo.
                      Jan. 2016: Surgery to remove residual mass. Pathology shows immature and mature teratoma.
                      March 2016: Markers rising. PET/CT shows 4 active mediastinal lymph nodes.
                      April-July 2016: 4xGAMMA (dactinomycin, HD methotrexate, oxaliplatin, paclitaxel).
                      July 2016: SBRT mediastinum, markers rising. PET/CT shows tumors in mediastinum, pleura and abdomen.
                      Aug.-Oct. 2016: HDC, 2xEC, markers normal, almost all tumors resolved.
                      Jan. 2017: Markers rising. PET/CT shows activity in mediastinum, pleura, abdomen. Oral etoposide and SBRT on active tumors. PD.
                      March-April 2017: cisplatin, gemcitabine. PD.
                      May 2017: pazopanib. PD.
                      June 2017: imatinib.

                        Have not posted for a while. My husband went through two IEP courses and AFP is normal now . Last IEP finished 8th of July. He noticed that his vision dropped a lot after last chemo, do you know anything about it?
