He had a complete response with chemo and the post chemo CT scan showed a residual mass.

While going through the process of choosing the surgeon, his AFP began to rise. Dr. Einhorn said this would be residual yolk sac tumor, and he was right.. Anthonys path report after RPLND was 80% teratoma 20% yolk sac ..

If there is residual cancer, but markers normalized during chemo .. how would that be catagorized? Relapse? Incomplete response? or chemo resistent?

In any case, is the follow up the same in this situation for stage 3 non-seminoma?

My call (and not that I'm not an M.D., I'm just going by the semantics here) would be an incomplete/partial response, since the markers did come back up prior to surgery, right? Chemo resistant would be if there had been no response to the treatment. Chemo resistant applies to the cells themselves.

The NCCN Guidelines note what the appropriate follow-up is.

Yes.. He finished chemo on April 6th and all markers were still normal. I think it was May 2nd when they checked his markers and it was 10.8 and from there it continued to rise.

I would classify Anthony's response as Incomplete. It dosen't really change anything other then his remission date should begin at the time of RPLND and his cure date will be two years after that.

Lots of appointments for a bunch of you! Best of luck to you all. I'm sure the anxiety doesn't get better. I don't care if it's the 2,3 5 or 10 yr. mark, the thoughts still go through your head. But you have all prevailed thus far. Prayers, hugs, love, coming at you all.

Jason's blood work came back with an "all clear" now it's the scans tomorrow. A bit of the joy has been taken out of the news because of Anthony's current problem. Let's hope the follow-up in the morning can shed some light on why his markers have gone up.

I am very happy to hear Jason's bloodwork is good, and I'm confident the scan will be as well.

The cage just tightened a bit. Because of some issues at work Jason postponed his scan until tomorrow. Let's just hope that he and Mark hit back to back homeruns.
