Yep. AFP can be even higher than yours and be fine. What you don't want is anything approaching 25, or a spike, Otherwise, enjoy the good news.

JUL01- Right orch.; 4x3 cm tumor, 90% EC, 10% yolk), CT shows 2.8cm met in abdom. + smallish mediastinum mets including one 7mm; staged III-A
JUL06- 3xBEP start
JUL07- AFP ~225, bHCG ~110, LDH ~2.3x, LDH qualifies as stage III-B, upped to 4xBEP
JUL28- AFP ~25, bHCG ~15, LDH normal
AUG17- AFP, bHCG, LDH normalized by 3rd cycle
SEP21- 4xBEP done, no complications; a few +2cm masses remain in retroperitoneum
DEC02- RPLND, necrosis + bit of mat. tera., retrograde
JAN. 2016- Surveillance, exercise, working, SIGUE!
NOV. 2017- Retrograde improves. Still not normal but better.
FEB. 2018- 26 MONTHS ALL CLEAR.
Dude, your story is quite similar to mine. I think yours is also primary retroperitoneal like mine as you didn't mention you had anything in testis. I think you are in good hands if you are treated by Prof Heidenreich. Have heard only good things about him here. I agree you have had a very good response to chemo if the pathology report came clear for you. I think RPLND was the right thing to do. You probably had a very good and experienced surgeon as he could do it in only 5 hours, it's very fast. Mine was 21 hours. Strange you have been suggested additional rounds after 4*BEP. It is definetly not a standard practice and I think Dr Einhorn would not recommend it. It was probalby a good decision from you to deny from it. I would probably follow Prof Heidenreich's recommendations in the future.
08-12/2015 Back pain, several epididymitis, fever, night sweats, went to many doctors but nobody thought I had cancer as I had nothing in testicles
01/2016 Retroperitoneal seminoma stage III, 28cm (tall) mass in retroperitoneum, big masses also in neck
02-04/2016 3*BEP
09/2016 Fever again -> relapse, cancer now in lungs too
09/2016 2*TIP
10/2016 1*HDC with stem cell transplant -> almost died to severe side effects, one week in ICU, 2nd round skipped
02/2017 RPLND (21 hours!) - one week in ICU again, found necrosis/fibrosis only!
06/2018 All clear (1,5 years now)
Happy to hear that you are doing well one year since being dx. The ALL CLEARs x 2 are the best news!!! And that you are active, and have gained some weight back.

Thanks all. The next step is a biopsis of my right testis if there are TIN-cells or not. Ultrasound showed that there is a "Sternhimmelsyndrome" (Sorry, don' know the english term for that)!

My left testis had the primary tumor and was removed. But the tumor was extremly small, but infiltrated the vein and lymphroad. (L1 and V1). I couldnt feel it , it was just found on the ultrasound.
The metas in my lymph nodes were very big, so they thought i had lymphoma. 1 Day later they found my BHCG was about 225.000, so they checked my testis.
I followed your whole story since my diagnosis and i am glad that you are doing well now! Seminoma metas are extremly sticky after chemo, so they are hard to remove, but 21hours is insane! Really glad, that you are back on your feet!

Congratulations, Martin! You are an inspiration! I hope the same for my son, who is still in limbo six months after chemo, because he still has one problematic lymph node at 1.2cm. Surgery is an option but so is waiting, we are told. You were brave to go through the surgery and get everything out.
