so...i just got path results and wondering if anyone had something
similar and what they're follow up treatment was....


Sections of testicle show malignant mixed germ cell neoplasm with predominantly embryonal carcinoma and yok sac tumor....the tumour is confined within the testicular parenchyma without involvement of tunica vaginalis. no lymphovascular invasion is identified....

diagnosis:

-malignant mixed germ cell with predominantly embryonal carcinoma and yolk sac tumour
-spermatic cord free of tumor

TNM classification: pT1NXMX

Hi Zoltar,

My son is mostly embryonal 95% and yolk sac 5% .. but the lymph is involved and a couple nodules in the lungs too.

He will be done with his third round of chemo tomorrow. One more round to go. After that, I am not sure if he will need more treatment. Possibly surgery.

Do you have the results from the blood work and scans? They are key in deciding if you need any future treatment.

well, scans are tomorrow and he said can't do the blood work for another 20 days...would the entire path report here be helpful?

20 days? That seems like a long time. If I'm not mistaken, the half-life of tumor markers is less than 10 days, and since you have embryonal and yolk sac, you need to make sure that your markers have gone down since your surgery. Your pathology report looks quite promising, but you need the bloodwork to fill in the holes. These results will be crucial in determining whether you will need further treatment.

When my son had his orchiectomy on Dec. 20th the urologist ordered new blood work to be done on January 20 something. A full month later.

We got it done a little bit sooner because we went to my sons primary to get a referral to an oncologist instead of waiting. The oncologist ordered new blood work, a CT scan, and set an appt. to get a port inserted to prepare for chemo to begin by Jan 29th.

I didnt know anything about TC at that time, but I still thought to myself .... from Dec 20 to late Jan for new blood work seemed like a long time.
As I learned more about TC I came to understand how precious time is, and how casual some doctors are about it.

well, to be honest...i thought 20 days seemed like a long time too but y'know i'll be seeing an oncologist early next week...so we'll see what he/she says. my afp was 67 going into surgery up from 34.5 (10 days prior).

the frustrating thing is both my urologist and pcp don't like the ONE oncologist that exists in my medical group/hmo plan. they say, well...he's really old and doesn't really probably keep on the latest stuff. so i'm preparing myself for a bigger battle. let's hope i can get this stuff resolved so it doesn't take another 10 days to see a decent oncologist. ugh..this is the worst part...and i have GOOD health insurance.

anyway...those are my levels and CAT scans today. appreciate the words thus far. thanks
