    I've had a dull ache in my remaining testicle for four days now, with today being the most pronounced. I have some transient pain in the low abdomen and down the thigh and in the back.

    Testicle seems unchanged to manual exam.

    I do not recall any activity that would cause this, though at first I assumed I just moved wrong or crossed my legs the wrong way or something. Now that it hasn't subsided and grown a bit more intense, I'm starting to pay attention.

    Anyone else experience this? Thoughts?

    I get the pain in the survivor, but I'm only 6 months out. Theory is that it's "working harder" to supply testosterone. I just had an ultrasound, because in addition to the discomfort (I wouldn't quite call it 'pain') I had some epididymal swelling. The US confirmed the swelling in the epi, and the testicle is a little bit larger, which is consistent with hypertrophy from "working harder".

    The back and abdominal pain is worrying, but I'm hyperaware of all kinds of pain. I have a constant numb feeling in my lower back, and sometimes down my thigh. I'd say that you should definitely get the US. It's required periodically, especially for seminoma. Good Luck!

      Hey Glenn,
      I've gotten scares of that sort a couple of times already; and yes, they are very freaky. I get particularly concerned because my treatment team and I already know that there is something inside lefty we don't fully understand (it's tiny, linear in shape, was picked up when I was first diagnosed, and hasn't changed since). I am mandated as part of my surveillance to get ultrasounds every 6 months until further notice. This is alongside doing TSEs regularly.
      My take would be to bring it up with your urologist directly, especially if it seems to be getting worse. Your doc will likely check it, and maybe will order an ultrasound to allay your concerns (I am very stubborn, so chances are I would insist on it). I would concur with the back pains; however, having a 2-year-old that wants Daddy to pick her up constantly throws the back pain out as a control experiment.

        Thanks, guys. It could be the imposter, who knows. I'm not to happy with it and probably wouldn't have gotten it if I knew what I know now.

        There's definitely nothing in the survivor remotely similar to his diseased, deceased brother. The increasing pain is worrisome, but I also ran yesterday. And, you know how it gets: at some point you don't know what is real pain and what's in your head.

        Any other thoughts out there?

          So how many of you are having US for the survivor that are Seminoma cases? I've had one US, and that was on the dearly departed.

          This pain is unique for me - not like any other pains I've had since the surgery, so that's what's alarming. The only place I can palpate the pain is a small spot just at the top of the genital area on the right side. Almost feels like a bruise. But the ache is in the scrotum/testical area with flashes in the other places.

          I could have aggravated some soft tissue. I have SI joint pain and stretch a lot for my golf game, and I did play twice this weekend. And I don't swing easy.

          I just expected it to subside...

            I think you should get another blood test for testosterone and this time add E2, your remaining testicle might be pooping out from all the hard work its been doing. I have had pain in my remaining testicle since my I/O and lowering my FSH from 12 to 7-9 has done nothing, I am suspecting it is high E2 now and going to get that sorted, quiet afew guys with testosterone deficiency have reported pain in there testicle and it subsided with the use of AIs such as nolvadex, clomid or arimidex.

            You should also be getting yearly ultrasounds on the remaining testicle.

              For the reasons I mentioned above, ultrasounds are a mandatory part of my surveillance. My next one is scheduled for June 1.

              The other pains that I have had are likely remnants from the I/O (neuroma formation from scarring of damaged nerves; fairly common after this particular surgery). These will likely be around for 6 months post-I/O, perhaps longer. Jogging seems to make these pains worse, but that doesn't happen all the time, so sometimes it just seems it's in my head, just like you say.

                Fed, I can definitely understand the USs in your case. This never came up in my surveillance plan.

                Michael, those are a lot of abbreviations I don't know! What's E2, FSH and AI?

                I had testosterone checked for several appointments because of depression that I was having. It was on the low side of normal, from 360-460. I haven't had it checked in several months.

                I didn't know I could overwork my testicles! I've been working them pretty hard for about 35 years now. I told my wife the pain was from lack of use. She told me to go take care of that. Gotta love it.

                My last surveillance check was in April - blood work (AFP and HCG) and a chest x-ray. In two weeks I get that and the abdominal CT. I may move those up depending on how this goes.

                I'm thinking a couple Vicodin with my gin and tonic tonight will help me with the pain and the worry

                  E2 is Estradiol which is the most potent of the 3 estrogens, you should have it checked to make sure its not within range but towards the lower end of the range, the optimal range would be between 10-30pg/ml or 37-104pmol/l

                  FSH is follicle stimulating hormone, it sends signals to the testicles to produce sperm, so when you lose 1, it naturly goes up usually out of normal range and that causes the testicle to grow in size.

                  AI is an aromatase inhibitor it inhibits the aromatase enzymes, aromatase enzymes convert androgens like testosterone to estrogens, if you have low testosterone you can get estrogen dominance which causes problems such as female breast tissue growth and a sore testicle among other problems.

                  Depression and joint pain is another 2 symptoms of testosterone deficiency.

                    Michael, great stuff. Are you a doctor, or just play one on TV?

                    Testosterone never dipped into the officially low region, and the breasts are still manly.

                    But, the survivor has grown in size. Not significantly, but it has grown. Which makes that measly fake one all the more small in appearance.

                    One other thought: I had a vasectomy on the survivor (poor guy) in March 2006. I then had another one in October because the vas grew back together. I'm wondering if that could be happening again?

                      Im not a doctor but have been doing my research

                      A complication of vasectomy can be chronic pain, so that gives you 2 possible reasons for the pain. Unless you had any tell-tail symptoms from the vas groing back together and your getting those symptoms again then I wouldnt really suspect it being the cause. You could look up on negative vasectomy experiences or "pain after vasectomy" and see if its simular to your case, I would look for how soon after the vasectomy the pain started and compare it to your situation.

                          So today I get up and, to my pleasant surprise, am pain free. I proceed to the bathroom and, when I urinate, I get a virtually identical pain in the abdomen prior to being diagnosed with TC. So, I went from one extreme to another.

                          I'm scheduled for a US first thing tomorrow morning. Frankly, I don't think it's TC on the right. I'd have to feel something in the testicle. We'll see.

                          Thanks for all the comments and care, it really helps. I am so fortunate to have this forum and to have had such a positive outcome to my cancer, so far.

                            Boring, wouldn't that be nice? Good luck tomorrow but I'm sure you're going to be ok.

                              Just had the US. Testical looks fine. Epididimus looked enlarged/swollen. They're saying probably epididimitus. Any experience out there with that? Pain is nearly all gone now.
