  • Cancer Survivor Care Plan

    Hello to all,
This is a very good article and document regarding cancer survivors. Inside of the article is information about nurses working collaberatively with patients to create a long term Care Plan with cancer survivors. The Care Plan is then a coordinated effort between all care that has occurred and as well a guide for long term future care. The survivor can keep the care plan and carry it for "any" furture medical needs. Not all doctors and nurses are expert at Oncology, but the guide can educate them for improved efficiency and direction for diagnosis and treatments as needed. The Care Plan is printable inside of the document. It is a very new concept, but Care Plans have been around for a long time. Nurses know what it is about. I am certain that it would be a welcome document to share with any care provider. Also, there are many helpful links within the site.

I double checked the site and it looks like they changed to location.
    For access also try
    Search Prescription for Living+Cancer Survivors
Select Nursing Center - Professional Development-CE Article
(There is a $29.00 charge, but that is only if the nurse wants to use this for educational credits. You can read the article for free.)
Will try to practice my computer skills and see if I can capture the document. S
