DATE OF VISIT: 09/20/2007
HISTORY OF PRESENT ILLNESS: It has been 2 years since the patient began
his chemotherapy for testis cancer. He is feeling very well and having no
difficulties whatsoever.
PHYSICAL EXAMINATION:
GENERAL: He looks great.
VITAL SIGNS: His blood pressure is 120/80, weight 113.2 kg, temperature
37.3, pulse 84, respirations 16.
LYMPH NODE SURVEY: No cervical, supraclavicular, or axillary
lymphadenopathy.
LUNGS: Clear to auscultation and percussion.
HEART: Normal heart sounds without gallops or murmurs.
ABDOMEN: His liver and spleen are not enlarged and there are no masses.
EXTREMITIES: No edema.
Today, he had a chest x-ray, which to my eye is free of active disease. We
also got an HCG and alpha fetoprotein.
This completes his 2 years of followup. We will see him again in 6
months.
